Bring a short account of your health.

Gather the details that help you tell the story. Use the notes as a prompt, not as a test you have to pass.

Make a short set of visit notes

Write your aim. Describe the concern and the change you hope to discuss in everyday words.

List your health details. Bring a list of what you take, including supplements, plus your health conditions and allergies.

Mark the open questions. Copy the unanswered questions from the offer, including the exact medicine name and full bill.

Keep the next task clear. Leave room to write what happens next and who will explain any results.

Bring what helps explain the concern

Add when you first noticed the problem, how it affects daily life and what you have tried so far. Plain notes are enough; there is no need to find medical words for them.

If you have results from earlier care, ask how to send them, such as by post or through the service’s website. Ask which records they need rather than sending every file.

For an online visit

Confirm your intended appointment location with the service, rather than assuming it from your home address. Ask what equipment the visit uses and how to reconnect if the call drops.

Make a place for your questions and the offer you are trying to understand. Ask how you can get a written account of the discussion afterward.

End with an account you can read back

Write down the next task, the person responsible and the way to ask a further question. If no treatment is agreed, ask what the next useful step in understanding the concern would be.

You can leave with a clearer question rather than a product. Keep the notes so a later discussion does not have to begin from memory.
